Medium-term outcomes

The 2023 ITI review included four prospective and two retrospective cohorts with at least five years of mean follow-up. It estimated 97.2% survival, about 1.1 mm mean marginal bone loss, and 3.0 mm probing depths. The sample was 277 implants, and cohort selection limits universal interpretation.

Comparative biological evidence

A 2026 systematic review of six comparative studies and 354 implants found no significant difference in reported biological complications between zirconia and titanium over 12 to 60 months. An isolated bleeding difference at 12 months was not accompanied by differences in other measured peri-implant outcomes; longer standardized trials remain needed.

Where evidence is expanding

Research now includes immediate loading, two-piece connections, fixed partial dentures, overdentures, and complete arches. A 2026 immediate-loading review rated evidence certainty low because of heterogeneity and predominantly nonrandomized designs. Restoration studies using zirconia over titanium fixtures must not be counted as evidence for zirconia implant bodies.
