How does the evidence differ?
Titanium implants have been studied for decades across large populations, many implant designs, and a wide range of restorations. This makes titanium the most evidence-established material overall.
A 2023 systematic review of commercially available zirconia implants estimated pooled five-year survival at 97.2% across six cohorts and 277 implants. The result is encouraging, but most included treatment involved one-piece implants supporting single crowns or short fixed restorations. It should not be generalized to every zirconia system or complex indication.1
Color, soft tissue, and esthetics
Zirconia’s white color may be attractive where gum tissue is thin or where a patient is concerned about gray show-through. Titanium can also produce excellent esthetic results when position, tissue, abutment, and restoration are managed appropriately.
Implant color is only one variable. Bone and tissue volume, smile line, implant position, abutment selection, and restoration design often have greater influence on the final appearance.
Biocompatibility and inflammation claims
Both materials are used because they can be biocompatible and support osseointegration. Laboratory findings about plaque, corrosion, particles, or cell response can help form research questions, but do not automatically prove one implant prevents disease better in patients.
Current evidence does not establish zirconia as universally superior for preventing peri-implantitis. Maintenance, smoking, diabetes control, implant position, restoration cleansability, prior periodontal disease, and plaque control remain important.
How should the choice be made?
The decision should connect material preference to anatomy, bite, implant position, restorative requirements, grafting needs, component design, available evidence, and the treating team’s experience. A patient may reasonably prefer a nonmetallic implant, while another may prioritize the widest component selection and longest evidence record.
Ask what exact system is proposed, why it fits the indication, what uncertainties apply, and what alternatives are reasonable.
